Scheduling & Visits
All programs begin with an in-depth nutrition analysis and lifestyle evaluation. Together we will do a deep dive into your health and eating history as well as review the comprehensive intake forms, and any relevant past lab work. We’ll also discuss your health struggles, your current dietary habits, goals, and desired outcomes. If not already part of your program, lab test recommendations will be provided and follow-up appointments scheduled.
The initial consultation is US$199.
Until further notice, all sessions will be done virtually via video conference utilizing our secure and HIPAA compliant health portal.
Clients have unlimited email access with our Longevity Functional Wellness team. However, if an email question or request is deemed to be too complicated or involved for a response via email we may, at our discretion, require that the client schedule a virtual visit instead of responding via email.
There is a 24 -hour cancellation policy for all appointments with Longevity Functional Wellness. Canceling within 24-hours will result in a $50 charge to the credit card on file.
No-shows: If you do not show up to a an appointment without notifying us in advance or rescheduling the appointment using the online scheduling tool on the client portal, you will be charged $100 to the card on file.
Billing & Insurance
Longevity Functional Wellness is currently in network with Aetna and CareFirst BlueCross in Maryland and United Healthcare and Medicare in Florida. We are in the process of adding additional insurance companies that we accept at our practice. Call us today to discuss your specific plan and options. In the event we are out of network, we can provide superbills for you to submit to your insurance on your own.
Longevity Functional Wellness is set up to accept HSA/FSA charges. Note: this does not guarantee a card will not be declined. The issuer of your card/plan has the authority to authorize or decline any payment.
- Some lab tests we offer as part of our functional medicine testing program may not be covered by insurance and in these cases you will be charged a fee for a particular test by the lab or through Longevity Functional Wellness.
- The specific tests ordered will be personalized to you based on your initial consultation or based on the program you purchased. These may include blood, saliva, urine, or stool testing.
- We will make every effort to make you fully aware of any additional lab fees that may be incurred and to give you the opportunity to choose which lab tests you feel comfortable paying for.
- You are under no obligation to purchase any specialty testing.
- At home test kits: If a lab test kit is mailed to you by Longevity Functional Wellness, you will be charged the day the kit is mailed.
